What Is an Person centered service plan (PCSP)?
A PCSP is developed by sitting down with you and your support team — family members, friends, community agencies, and your case manager — to identify what your needs are and outline the best way to meet them.
Your plan can include things like employment opportunities, vocational options, staffing supports, housing needs, and community integration. If special needs require funding, we can apply to state and local government agencies on your behalf.
Once your plan is in place, we don’t disappear. We contact you on a regular basis, review your progress, and adjust goals at any time. If something unexpected comes up, we can help you through a difficult time or crisis.
